Davis Vision Supplemental Vision Care Benefit

We are pleased to announce that you and your dependents are eligible to enroll in a voluntary program sponsored by AFSCME Local 4041. Davis Vision, one of the nation's leading vision care companies, will be administering the plan.

This plan will allow you access to an extensive network of licensed providers where, through pre-tax salary deductions, you may pre-fund your benefit and realize tremendous savings. Once you have elected coverage, every 12 months you will be entitled to a comprehensive eye examination, including dilation as needed, and spectacle lenses or contact lenses in lieu of eyeglass lenses. Every 24 months you will be entitled to your choice of a selection of designer eyewear (approximately 300 first quality frames to choose from.)

Your Coverage under the plan will include:

A comprehensive eye examination, including dilation when necessary
A frame from the Designer selection
Single vision,bifocal and trifocal lenses (plastic or glass) in any prescription range
Over-size lenses
Glass-grey #3 prescription lenses
Fashion, sun or gradient tinting on plastic lenses
Post-cataract (lenticular) lenses
Polycarbonate lenses for children and monocular members

Receiving services is as easy as placing a call to one of the participating offices and scheduling an appointment. You will not receive a member card so when booking your appointment,give the doctor your social security number to verify coverage. The doctor will be responsible to obtain any necessary authorization(s). Please refer to the enclosed Summary Plan booklet for more information on the plan.
